ROC Sonoma County has established a team of Disaster Case Managers who will meet with clients on an individual basis. This initial meeting will be used to determine eligibility and what support ROC partners can provide. The first step will be to complete a needs assessment, which consists of a brief set of questions and basic demographic information. This needs assessment will allow case managers to identify your individual or family’s long term recovery needs. If eligible, then a dedicated Disaster Case Manager will be assigned to assist you throughout the recovery process. If you or your family are not in need of a dedicated Disaster Case Manager, then ROC partners will take this opportunity to identify other services that could be helpful to you and the community.
